Choledochoduodenostomy

Abstract
|
|
|
This is a clinical review and evaluation of 24 cases with Choledochoduodenostomy, which were done at the Department of Surgery, Pusan National University Hospital during the past 10 years from 1981 to 1990.
The results were as follows.
1) Common indications for choledochoduodenostomy were intrahepatic with common bile duct multiplestones(23.5%), multiple common bile duc stones (15.9%) and malignant diseases(3L6%).
2) Pre-existing complications were cholangitis(12.5 % ), gallbladder empyema (8.3%), and liver abscess(42 % ).
3) Average diameter of common bile duct was 23.5mm and a diameter of stomy site was over 25mm in all cases and performed by on layer sutures.
4) Post-operative complications occured in 14 cases(58.3%). Common complications were wound infection(12.2 %), atelectasis(8.3x), and urinary infection (4.2 % ).
5) Follow-up studies were available for 16 patients (66.7%) and result was excellent in 6 cases (37Z%), good in 5 cases (3L3%), fair in 3 cases (18.8%), and poor in 2 cases (12.4%).
6) This review suggests that more aggressive and effective procedure should be taken for intrahepatic stones, however in poor risk cases, choledochodoudenostomy is considered as acceptable procedure.
